Insurance & Financing

At Evergreen Eye Center, our goal is to provide the highest standard of care, including when it comes to keeping you informed about insurance, billing and financial concerns. Read on to find out more about our policies and practices.

Insurance at Evergreen Eye Center

Evergreen Eye Center works with most major health insurance and vision plans across the Puget Sound area, for everything from a routine exam to cataract or retinal care. Coverage details vary
by plan, so we recommend confirming specifics directly with your carrier. Patients enrolled in an HMO plan should expect to bring a referral from their primary care provider to their visit.

Not sure whether your plan covers what you need? Call 206-672-2277 before your visit and our billing team can check your benefits and walk you through what to expect at check-in.

Insurance Terms, Explained

These are general definitions meant to help you follow along, not a substitute for the details of your specific plan. When in doubt, your insurance carrier has the final word.

Plan Type
What It Means for Your Visit
MEDICARE / MANAGED CARE PLANS
If your coverage comes through a Medicare Advantage or Medicaid managed care plan, the card you hand our front desk may actually be from a private carrier, such as Blue Cross Blue Shield or United Healthcare, rather than a red, white, and blue Medicare card. Let our team know if you're enrolled in a managed care plan so we can bill it correctly.
PPO
A PPO plan lets you choose your own provider without a referral. Staying in network keeps your copay, co-insurance, and deductible at their standard rate. Going out of network usually means paying more out of pocket.
HMO
An HMO plan requires a primary care provider who refers you out to specialists like us. Coming in without that referral on file typically means the visit isn't covered, and the cost becomes your responsibility, so bring your referral paperwork with you.
Vision Benefits
Some health plans include a separate routine vision benefit, often handled by a different administrator than your medical coverage. It's worth checking who manages that piece of your plan specifically, since it won't always show up alongside your regular health benefits.
Deductible
The amount you pay out of pocket before your insurance starts covering its share of a service.
Co-Pay
A fixed amount due at the time of your visit, set by your plan.
Co-insurance
Your percentage of the cost after your deductible has been met, as opposed to a flat fee.
